In essence Chris' idea is the same as the guide. There is a drop of threadlock on the knob, thus making it rather tight and hard to unscrew, once you managed the initial twist-off, the rest should be easy. You deffo need to hold the gear shft stable, because if you don't you might end up damaging the syncro-mesh (read as "cause expensive fix")
